Use

Use to record summary information about a functional impairment affecting an individual's ability to perform activities or participate in life situations.

This archetype is one of a set of three related archetypes designed to represent different aspects of how an individual functions in everyday life. Together, these archetypes support structured representation of functional ability, impairments contributing to functional limitations, and the supports required to enable or assist an individual to perform an activity. The other archetypes are:

  • OBSERVATION.functional_ability and
  • EVALUATION.functional_support_requirements.
MisuseNot to be used to record an assessment of the range of supports required to optimise an individual's ability to perform an activity or participate in a social situation. Use the EVALUATION.functional_support_requirements archetype for this purpose. Not to be used to record an assessment of functional ability. Use the OBSERVATION.functional_ability archetype for this purpose. Not to be used to record information about a problem or formal diagnosis. Use the EVALUATION.problem_diagnosis archetype for this purpose. Not to be used to record information about a symptom or physical sign. Use the CLUSTER.symptom_sign archetype for this purpose.

The value set is aligned with the ICF 'Extent or magnitude of impairment' qualifier scale. The value 'No impairment' may be recorded when a previously identified functional impairment has resolved or is no longer affecting function.

    • No impairment [None, absent, or negligible (0–4%).]
    • Mild impairment [Slight or low (5–24%).]
    • Moderate impairment [Medium or fair (25–49%).]
    • Severe impairment [High or extreme (50–95%).]

ImpactImpact: Narrative description of how the impairment affects the individual's ability to perform specific activities and participate in everyday life situations.
For example: Requires written reminders and assistance to follow multi-step instructions and manage medications independently.
TrajectoryTrajectory: The overall trend in the severity and impact of the impairment over time.

    • Stable [The overall trend in the severity or impact of the impairment over time is unchanged; no improvement or worsening noted over time.]
    • Improving [The severity or impact of the impairment is decreasing over time.]
    • Worsening [The severity or impact of the impairment is increasing over time.]
    • Fluctuating [The severity or impact of the impairment varies over time, with periods of improvement and worsening.]
    • Unknown [The overall trend in the severity or impact of the impairment over time cannot be determined.]

Date of onsetDate of onset: The date on which the impairment was first identified as affecting function.
Partial dates are permitted.
Date of resolutionDate of resolution: The date on which the impairment resolved or was no longer affecting function.
