Use

Use to record the individual’s current pregnancy-related care state so that relevant pregnancy considerations can be taken into account in clinical decision-making. The state may be reassessed and updated over time as clinical circumstances change, including before, during and after pregnancies.

The defined pregnancy care states represent distinct pregnancy-related circumstances across the reproductive continuum. These include a default baseline state, pregnancy-related physiological states, intent to conceive, and situations where pregnancy care does not apply. Each of the following mutually exclusive care states reflects circumstances in which pregnancy considerations may influence clinical management:

  • 'Premenarchal' - applies when pregnancy is generally considered physiologically unlikely because menarche has not yet occurred. Clinical judgement may be required when determining the transition between 'Premenarchal' and 'Baseline' states during puberty, as ovulation may occur before the first recognised menstrual period and pregnancy may therefore be possible prior to documented menarche.
  • 'Baseline' - the default pregnancy care state used when none of the other defined states apply. In this situation, routine pregnancy-related clinical safety considerations should be applied because pregnancy may be unrecognised.
  • 'Preconception' - applies when the individual is actively planning or preparing for pregnancy, such as intentional discontinuation of contraception, preconception clinical optimisation, or onset of fertility treatment, and pregnancy-related care requirements should be escalated beyond the 'Baseline' state to support safe conception and early pregnancy.
  • 'Pregnant' - applies when pregnancy has been clinically confirmed and pregnancy-specific care requirements apply.
  • 'Post-pregnancy' - applies during the recovery period following the end of a pregnancy, regardless of gestational age or outcome, during which 'Baseline' pregnancy-related care requirements and other post-pregnancy care considerations apply.
  • 'Not applicable' - applies when pregnancy is biologically impossible and pregnancy-related care considerations are not relevant, for example when the individual is post-menopausal or does not have a uterus.

Typical events that may trigger an update to the 'Pregnancy care state' include, but are not limited to:

  • Documentation of first menstrual period (menarche): transition from 'Premenarchal' to 'Baseline'.
  • Documentation of fertility treatment commencing: transition from 'Baseline' to ‘Preconception’.
  • Confirmation of a pregnancy diagnosis: transition from 'Baseline' or 'Preconception' to ‘Pregnant’.
  • Documented birth or pregnancy loss: transition from 'Pregnant' to ‘Post-pregnancy’.
  • Completion of the immediate postpartum period: transition from 'Post-pregnancy' to ‘Baseline’.
  • Documentation of menopause or a hysterectomy procedure: transition from 'Baseline' to ‘Not applicable’, which is usually considered a permanent state.
MisuseNot to record pregnancy confirmation or exclusion at a specific point in time. In particular, a clinician's determination that an individual is ‘not pregnant’ is only valid as a point-in-time clinical assertion and should not be used to represent an ongoing pregnancy care context. Use the OBSERVATION.pregnancy_assertion archetype for this purpose. Not to be used to record a suspicion of, or level of uncertainty about, a diagnosis of pregnancy, such as 'possible', 'probable', 'suspected', or 'confirmed'. Use the EVALUATION.problem_diagnosis for this purpose. Not to be used to record the summary information about a single pregnancy. Use the EVALUATION.pregnancy_summary for this purpose.
