| Description | Provision of a health assessment required to evaluate an individual’s physical, mental, developmental, or functional health status, for the purposes of prevention, screening, diagnosis, or care planning. |
| Use | Use to record details about the provision of a health assessment at clinically relevant events in the intervention lifecycle, from planning to completion, and including deviations or interruptions. The scope of this archetype is focused on the provision of any health assessment to the individual, including by other service providers. It includes, but is not limited to:
Record one instance per careflow step or event for each specified health assessment, in a health record. If the assessment to be provided consists of a single component, the 'Assessment provided' and the 'Assessment completed' careflow step may be recorded simultaneously. However, if the assessment consists of multiple components, record a separate instance of the 'Assessment provided' careflow step for each component delivered, and record the 'Assessment completed' step only after the final component has been provided. |
